Applications are available at the Clerk of Court Office for both regular and renewal of passports. Appointments are not required, but recommended.

Please bring:
  • A certified copy of your birth certificate
  • 1 passport photo
  • Your driver's license or ID card
The applicant must appear in person. Two separate forms of payment are required for passports. The first payment is by check on only and is to be made payable to the Department of State. The second payment is payable to the Clerk of Court and can be in the form of check, cash or money order.

Passport Renewal
Renewal of passports can be done through the mail as long as all the requirements that are stated on the front of the form are met. Please call the Clerk of Courts Office (608-685-6212) with any questions, or check the website.
